Host genetics can significantly influence an individual’s susceptibility to HIV infection and the course of disease progression. Among the genetic factors identified to date, the CCR5-Δ32 mutation is the most extensively studied and clinically relevant variant associated with natural resistance to HIV-1. Individuals who inherit two copies of this mutation are highly resistant to infection by most HIV strains, while those carrying a single copy often experience slower disease progression following infection.

Beyond its role in HIV susceptibility, CCR5-Δ32 has become an important consideration in hematopoietic stem cell transplantation (HSCT) for HIV-positive patients with hematological malignancies. The landmark case of the Berlin Patient demonstrated that transplantation from a donor carrying the homozygous CCR5-Δ32 genotype can lead to long-term HIV remission and potentially cure.
